Notice of Intent to Relocate Forms Available

Court forms related to a parent filing a Notice of Intent to Relocate are available on the Court’s website. The Notice of Intent to Relocate has been revised and several new forms have been created. These forms comport with state law regarding a parent’s right to receive notice of the other parent’s Notice of Intent to Relocate while also providing confidentiality where necessary.

These forms include:

• Notice of Address (Entitled)

• Notice of Intent to Relocate R.C. 3109.051(G) (Not Entitled to Notice)

• Notice of Intent to Relocate R.C. 3109.051(G) (Entitled to Notice – Address Confidential)
• Notice of Address (Entitled - Address Confidential)

• Motion for Order for Non-Relocating Parent Not to Receive Notice of Intent to Relocate
