Judgment Entries

Our forms are offered to give you an idea of what Domestic Relations Court documents look like. They can be printed for your own personal use. Blanks are indicated where information is to be inserted. Please remember that preparing the appropriate forms with the appropriate information is only the first step in the Court process. If you are representing yourself, you will still have to become knowledgeable of the applicable Ohio Rules of Civil Procedure, Ohio Rules of Evidence, all applicable statutory laws and Local Rules of Court. Please read all instructions carefully.

 

ATTENTION: Attorneys are required to submit judgment entries for preapproval two weeks before the hearing to: drpreapprovals@cuyahogacounty.us.

In cases involving minor children, attorneys must also file the following documents with the Clerk of Courts two weeks in advance:

  • Parenting Class Certificate
  • IV-D Application
  • Parenting Proceeding Affidavit
  • Health Insurance Affidavit
  • Military Service Affidavit (if applicable)
  • Relocation Affidavit
